Agreement to Terms

By accessing or using the Mental Chec website and services, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.

Services Description

Mental Chec provides telehealth mental health therapy services to individuals residing in the state of Florida. Our services are provided by Oksana Salaam, a Licensed Mental Health Counselor (LMHC) in Florida.

Eligibility

To use our therapy services, you must:

• Be at least 18 years of age (or have parental/guardian consent if a minor)
• Be a resident of Florida
• Have the legal capacity to enter into a binding agreement
• Provide accurate and complete information

Not Emergency Services

Mental Chec does not provide emergency services. If you are experiencing a mental health emergency, are in crisis, or are having thoughts of harming yourself or others, please:

• Call 911 for immediate emergencies
• Call or text 988 (Suicide & Crisis Lifeline)
• Text HOME to 741741 (Crisis Text Line)
• Go to your nearest emergency room

Telehealth Consent

By using our telehealth services, you acknowledge and consent to receiving mental health services via electronic communication. You understand that:

• Telehealth involves the use of electronic communications to provide services remotely
• There are potential risks including technology failures and privacy concerns
• You have the right to withdraw consent at any time
• Telehealth may not be appropriate for all conditions or situations

Appointment and Cancellation Policy

Appointments are scheduled through our booking platform. We request that you provide at least 24 hours notice if you need to cancel or reschedule an appointment. Late cancellations or missed appointments may be subject to fees.

Payment Terms

Payment is due at the time of service unless other arrangements have been made. We accept various forms of payment as indicated on our booking platform. You are responsible for any fees associated with your therapy services.

Confidentiality

We are committed to maintaining the confidentiality of your information in accordance with applicable laws, including HIPAA. However, there are legal exceptions where disclosure may be required, such as:

• Risk of harm to yourself or others
• Suspected abuse or neglect
• Court orders or legal proceedings
• As otherwise required by law

Intellectual Property

All content on this website, including text, graphics, logos, and images, is the property of Mental Chec and is protected by copyright and other intellectual property laws. You may not reproduce, distribute, or create derivative works without our express written permission.
